CHA PACE is a Program of All Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE"s high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in home support services.

The Clinical Social Worker is a member of PACE Interdisciplinary Team who assesses and provides for the behavioral health and psychosocial needs of PACE participants. They provide regular assessments, individual counseling and referral to community resources as needed.

Qualifications:

Master"s Degree in Social Work from an accredited school of social work.

LCSW/LICSW required

Experience in geriatrics, working with people with disabilities is highly desired

Experience in medical social work or integrated care highly desired

Experience working with diverse populations highly desired

Experience working as part of an interdisciplinary team strongly preferred
