Your Role on the Team Position Summary: The incumbent is a patient educator-advocate who empowers patients to develop self-management skills with their chronic disease such as diabetes and heart failure through improved communication, education and empathetic coaching. The goal of the community coach is to help patients and their families or significant others to cope with their condition, manage symptoms and improve function and quality of life. Provides and/or supports health care services to individuals with these chronic diseases at UC Irvine HealthCare primary care ambulatory and Cardiology sites as part of the healthcare team.

Required Qualifications: - Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
- Must demonstrate customer service skills appropriate to the job
- Knowledge of Diabetes or Heart Failure disease
- Interest in the community served by UC Irvine Health (A Passion for Caring)
- Effective communication skills
- Ability to read and communicate effectively in English
- Ability to maintain a work pace appropriate to the workload
- Ability to establish and maintain effective working relationships across the Health System
Preferred Qualifications: - Knowledge of University and medical center organizations, policies, procedures and forms
- Bilingual skills in English and Spanish and/or Vietnamese
