**Position Overview**
We are seeking a **compassionate and dedicated Nurse Practitioner (NP)** to join our team and provide **telehealth consultations and in-home visits** for senior patients. This role is ideal for a **new graduate** looking to build a rewarding career in senior care while working in a flexible environment.
**Key Responsibilities**
- Conduct **telehealth assessments and consultations** for senior patients.
- Perform **home visits** for patients requiring in-person evaluations and follow-ups.
- Diagnose and manage acute and chronic health conditions, focusing on geriatric care.
- Develop and implement individualized **care plans**, including medication management.
- Educate patients and caregivers on **chronic disease management, medication adherence, and wellness strategies**.
- Collaborate with **primary care physicians, specialists, and care coordinators** to ensure comprehensive patient care.
- Use electronic health records (**EHR**) to document patient encounters and treatment plans.
- Provide recommendations for **preventative care, screenings, and lifestyle modifications** to improve patient outcomes.
- Participate in **virtual case discussions** and team meetings to enhance care coordination.
**Qualifications**
- **Graduate of an accredited Nurse Practitioner (NP) program** with a Master’s or Doctorate in Nursing.
- **Board-certified** as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP).
- **Active and unrestricted NP license** in California.
- DEA license or eligibility to obtain one.
- CPR/BLS certification.
- Strong **interest in geriatric care, chronic disease management, and telehealth services**.
- Ability to work **independently and travel within the San Diego area** for home visits.
- **Tech-savvy** with experience using telehealth platforms and EHR systems (training provided if needed).
- Excellent **communication and bedside manner**, with a passion for improving senior healthcare.
