Job Title: Wound Care Nurse (RN or LPN)
Facility: Bedrock Rehabilitation and Nursing Center at West Volusia
Location: Deltona, FL
Shift: Days
Pay Rate: $35-$40 per hour (depending on experience)
Job Description:
Bedrock Rehabilitation and Nursing Center at West Volusia is seeking a dedicated and compassionate Wound Care Nurse (RN or LPN) to join our team. In this role, you will work closely with the interdisciplinary team (IDT) and healthcare professionals to ensure the best care for our residents.
Key Responsibilities:
Consult and coordinate with the IDT to plan, implement, and evaluate individualized resident care plans.
Participate in completing sections of the Minimum Data Set (MDS) assessment as needed.
Review and update policies and procedures related to skin and wound care.
Report any suspicion of crime or knowledge of fraud, abuse, or waste to the Administrator or designee.
Identify, manage, and treat primary and secondary wounds, including pressure ulcers, ostomies, surgical wounds, traumatic wounds, and burns.
Help mitigate risk factors for secondary wound infections and identify specific resident risk factors for pressure ulcers.
Assist the IDT in managing pain related to wounds and complete regular skin assessments.
Provide treatments and care instructions to residents and families in an understandable format.
Evaluate the nutritional needs of residents with healing wounds.
Make recommendations on pressure-relieving devices, therapies, dressings, and wound care products to the interdisciplinary team.
Qualifications:
Current unencumbered active license to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Florida.
Minimum of two (2) years of experience as an RN or LPN.
