Job Title: Case Manager
Location: White Plains, NY

Type: Contract To Hire
Compensation: $45k Annually or Hourly Equivalent 
Contractor Work Model: Onsite
Hours: 8:30a-5p, Mon-Fri

Overview:
We are seeking a compassionate and detail-oriented Mobility Program Counselor to support a community-based housing program in partnership with HCR. In this role, you will be the first point of contact for families and seniors referred by housing specialists, providing remote intake assessments and motivational counseling to help participants move into opportunity areas. This is a non-Section 8 role focused on supporting participants beyond traditional housing programs.

Key Responsibilities:

• Conduct remote intake surveys and assessments to understand family needs

• Provide empathetic, customer-focused counseling and resource referrals

• Maintain accurate participant records and data entry in SharePoint

• Collaborate with internal teams to support participants' Personalized Action Plans

• Participate in regular check-ins and progress meetings

Qualifications:

• Minimum 4 years of direct case management or counseling experience (community work, non-profit, or health services background preferred)

• Strong interpersonal and customer service skills, especially with vulnerable populations

• Proficient with Microsoft Office and comfortable with data systems

• Bilingual in Spanish strongly preferred
