Job Description

We are hiring a Social Worker for one of our clients in Wayne, NJ.

Job Description:

• Perform resident assessments.
• Provide resident-centered care as a member of the interdisciplinary care team.
• Plan for resident discharges and maintain required documentation.
• Provide counseling and guidance to residents and their families.
• Provides information on outreach services offered to families.
• Ability to establish and maintain cooperative relationships with clients and staff.
• Long-term care experience Social Worker in a nursing home facility. Must be able to assess/document in MDS.

Certification required:

• LSW, MSW.

Working Hours:

• 9am-5pm or 10am-6pm
• Lunch period: 1/2 hour Unpaid
